Enrolment Method
參加者須填妥表格上所有資料，
(1) 將捐款劃線支票 (抬頭 : 香港自助旅行者協會)， 連同報名表格(可在網頁下載)，
寄回 “香港自助旅行者協會”， 彌敦道610 號荷理活商業中心 1117 室， 或 (2)
將款項存入匯豐銀行戶口 a/c: 405-061391-001， 抬頭 「香港自助旅行者協會」， 連同報名表格， 寄回
「香港自助旅行者協」 彌敦道610 號荷理活商業中心 1117 室。
請於支票或存款單正本背面清楚列明參加者姓名、 身份證號碼、地址及聯絡電話 ， 資料不足或未付上所須支票的表格, 恕不受理。

Application: Fill up the application
form which can be downloaded from the Internet
(1) send together with the crossed cheque payable
to "Hong Kong Backpackers’ Association", ( Room 1117
Hollywood Plaza, 610 Nathan Road, Mongkok, Kowloon) Or (2)
transfer the payment to HSBC bank account: 405-061391-001 payable
to ‘Hong Kong Backpackers’ Association’. Mail the photocopy
of the pay-in-slip to ‘Hong Kong Backpackers’ Association’ with
your application form to Room 1117 Hollywood Plaza, 610 Nathan
Road, Mongkok, Kowloon.
Please write clearly the name of participant, identity card
no., contact address and contact no. at the back of the cheque
or cash deposit slip. Incomplete Application form or default
cheque will not be processed. ( for enquiries, please contact

註 Note：
1. 每位參加者必需籌募HKD 380 以上, 才可參加這次活動。
Each participant must collect at a minimum of HKD 380 in order
to join this event.
2. 請於支票, 存款單正本背面清楚列明參加者姓名、 身份證號碼、地址及聯絡電話。
Please write clearly the name of participant, identity card no.,
contact address and contact no. at the back of the cheque or cash
deposit slip.
3.請將填妥之贊助表格及劃線支票/存款正本於2004年7月26日或之前寄回“香港自助旅行者協會” 彌敦道618 號荷理活商業中心
1117 室。
Please send the crossed cheque/ photocopy of the pay-in-slip with
the complete application form to ‘Hong Kong Backpackers’ Association’,
(Room 1117 Hollywood Plaza, 610 Nathan Road, Mongkok, Kowloon)
on or before 26th July, 2004.
